I believe at one time, [human in the loop] was a good-enough phrase. Human in the loop earned its reputation in a slower world: one decision, one reviewer, enough time to think.
Agentic systems break that pace. Your agent acts in seconds. Your governance runs on meetings, approvals, and legal review, which take hours or days. Jason Bloomberg, in a May analysis at SiliconANGLE, calls this governance lag. A system can go badly wrong while your people are still trying to get the meeting on the calendar.
